Job summary

Canopy Children's Solutions in McComb, MS is seeking a Wraparound Facilitator to engage families and build a strong wraparound team to develop a holistic, family-centered plan of care. You will coordinate resources and supports to address complex mental health needs while maintaining fidelity to wraparound practice.

The role requires a social work background, the ability to travel using a personal vehicle, and a flexible schedule including evenings and weekends.

Qualifications

• Bachelor’s degree in Social Work or related field required.
• Must complete PCCSS trainings and certification exam within 90 days of hire.
• Must complete MM Health Community Support Specialist training within 90 days of hire.
• Flexible schedule including some evenings and weekends.

Responsibilities

• Engage family to initiate high-fidelity Wraparound.
• Develop family story and vision for the first 30-day wraparound meeting.
• Complete initial Crisis Management Plan within 24 hours of admission.
• Maintain team cohesiveness and positive family-team communication.
• Facilitate Wraparound Team Meetings at least every 30 days.
