Job summary

Bluebonnet Trails Community Services is seeking a Wraparound Facilitator to coordinate services across multiple systems and monitor progress toward outcomes for youth and families in the YES Waiver program.

The role emphasizes documentation in the electronic health record, performance with a family-driven, youth-guided framework, and collaboration with multidisciplinary teams. Evening hours may be required to meet families’ needs.

Qualifications

• Bachelor's degree in Human Services required.
• Strong understanding of mental health diagnoses and treatment.
• Ability to work independently and collaboratively in a fast-paced, recovery-oriented environment.
• Valid Texas driver's license required, with ability to maintain approved driver status.
• Candidates whose degree is not in Human Services must have completed at least 24 credit hours in a human services-related field.
• Master's degree in Human Services or related field preferred.
• Bilingual proficiency in Spanish highly valued.
• Current licensure as a Licensed Chemical Dependency Counselor (LCDC) is desirable.

Responsibilities

• Coordinate services across multiple systems and monitor progress toward outcomes.
• Advocate for youth and families and ensure continuity of care during enrollment.
• Maintain timely and accurate documentation in the electronic health record.
• Deliver direct wraparound services while participating in supervision, training, and quality improvement initiatives.
