Job summary

Integrated Support and Facilitation in Indianapolis, IN seeks a Wraparound Facilitator to coordinate individualized care plans for youth and families. You will work with families, caregivers, and partners to foster stability and lasting change in the community.

Responsibilities:

• Facilitate wraparound team meetings with families, caregivers, and community partners to develop and implement individualized care plans
• Build trusting, strengths-based relationships with youth and families to identify needs, goals, and natural supports
• Coordinate services and resources across multiple systems including behavioral health, education, child welfare, and community organizations
• Monitor progress toward family goals and adjust plans of care as needed through ongoing assessment
• Maintain accurate and timely documentation, case notes, and service records in compliance with agency standards
• Advocate for families to ensure access to appropriate services and community-based supports
• Collaborate with a multidisciplinary team to ensure holistic, family-driven care

Requirements:

• Bachelor's degree in Social Work, Psychology, Human Services, or a related field required
• Experience working with at-risk youth, families, or in case management/care coordination
• Knowledge of the wraparound process and trauma-informed, family-centered care principles
• Strong communication, active listening, and conflict resolution skills
• Ability to work flexible hours, including some evenings, to meet family needs
• Valid driver's license and reliable transportation required
• Proficiency in documentation and basic computer skills
