Job summary

Integrated Support and Facilitation in Indianapolis, IN seeks a Wraparound Facilitator to coordinate individualized care plans for youth and families. You will work with families, caregivers, and partners to foster stability and lasting change in the community.

Qualifications

• Strong communication, active listening and conflict resolution skills.
• Experience working with at-risk youth, families, or in case management/care coordination.
• Knowledge of wraparound processes and family-centered, trauma-informed care principles.

Responsibilities

• Facilitate wraparound team meetings with families and partners to develop and implement care plans.
• Build trusting relationships with youth and families to identify needs, goals and supports.
• Coordinate services across behavioral health, education, child welfare and community orgs.
• Monitor progress toward family goals and adjust plans as needed through ongoing assessment.
• Maintain accurate documentation and service records in line with agency standards.
• Advocate for families to access services and community supports.
• Collaborate with a multidisciplinary team to ensure holistic, family-driven care.
