Job summary

The ideal candidate should hold a Bachelor's Degree in a human service field and possess strong communication skills. Responsibilities include coordinating support services, conducting home visits, and ensuring accurate documentation. Qualifications

• 2-4 years of experience in Case Management and/or Outpatient/Inpatient behavioral health setting preferred.
• 1-2 years of experience in leading teams preferred.
• Must be able to work independently and without supervision at times.

Responsibilities

• Provide wraparound services to families with a limit of up to 15 members/families.
• Coordinate a comprehensive set of supports and resources for each family.
• Lead the planning team through engagement and plan implementation.
