Counseling and Research Associates, Inc. is seeking a Full-time FSP-HFW Facilitator to coordinate family-driven, youth-guided, culturally responsive services for children with complex behavioral health needs in Norwalk, CA.

You will identify and lead the Child and Family Team, develop individualized Plans of Care, monitor progress, and coordinate with schools, healthcare providers, and community partners. Strong crisis response and after-hours coordination are essential.

Qualifications

• Facilitate Child and Family Team (CFT) meetings with families and partners.
• Promote active child/youth and family involvement in planning.
• Coordinate across schools, healthcare, and community services.

Responsabilités

• Lead CFT meetings and engage natural supports and service providers.
• Develop and implement an individualized Plan of Care with defined goals and timelines.
• Monitor Plan of Care and adjust as needed to improve outcomes.
• Provide crisis prevention, de-escalation, and post-crisis follow-up within scope.
• Assist with after-hours crisis access and on-call coordination.
• Support co-occurring services integration for youth with complex needs.
