Qualifications

Responsibilities

• Provide direct clinical services and wraparound coordination within the county.
• Perform linkage, referral, crisis management, and follow-up activities.
• Supervise wraparound providers and volunteers; oversee documentation and reporting.
• Coordinate family teams and monitor progress toward individualized plans.
• Inform the community and stakeholders about wraparound services and access.
