Responsibilities

• Maintain a caseload of about 8 children and families.
• Contact enrolled families within 48 hours; set up initial visit within 5 days.
• Schedule weekly visits; facilitate monthly Child and Family Team Meetings.
• Assemble a Child and Family Team within two weeks of enrollment.
• Develop initial Plan of Care within 30 days, including a 24-hour Crisis Plan.
