The position requires a Bachelor’s degree in a related field and at least four years in behavioral health, with a preference for 5150 certification.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Human Services, Counseling, Sociology, Behavioral Sciences, or a related field.
• Minimum of four years of experience in behavioral health, social services, child welfare, case management, or a related field.
• Up to two years of graduate education may be substituted on a year-for-year basis for experience requirements.
• Experience coordinating care for individuals with complex behavioral health needs preferred.

Responsibilities

• Coordinate services and supports for clients in the High Fidelity Wraparound (HFW) Program.
• Serve as primary contact for clients and families, developing care plans and connecting to resources.
• Support long-term success within homes, schools, and communities; comply with policy and law.
