Job summary

Affinity HealthCare Solutions is seeking a skilled Licensed Practical Nurse (LPN) to provide Wound VAC therapy and wound care for home health patients. This ongoing assignment requires strong wound care skills and independent working abilities.

The LPN will perform dressing changes, conduct assessments, and provide patient education. Candidates must hold an active Pennsylvania LPN license and have Wound VAC experience. The role requires availability every Wednesday and Friday.

Qualifications

• Active Pennsylvania LPN license in good standing.
• Wound VAC experience required.
• Previous home health or facility-based wound care experience strongly preferred.

Responsibilities

• Perform Wound VAC dressing changes and equipment management.
• Conduct wound assessments and report changes to RN Case Manager.
• Provide education to the patient regarding wound care.
