Job summary

Pathways Healthcare is seeking an experienced home health nurse to join our in-home care team serving Boston and Southshore. The role emphasizes preventive and rehabilitative nursing while collaborating with physicians and interdisciplinary teams.

The ideal candidate will hold an active MA nursing license, WOCN certification, and have at least 2 years in geriatrics. BLS CPR, a valid driver's license, and compassionate bedside manner are essential.

Qualifications

• Must hold an active nursing license in the state where the position is based.
• Must hold an active Wound and Ostomy Certification (WOCN).
• Preferably 2+ years of nursing experience with geriatrics focus.
• Nursing degree from an accredited institution.
• Maintain current BLS CPR certification.
• Possess a valid driver’s license and own a reliable, insured vehicle.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather medical histories.
• Develop and revise individualized care plans based on patient status.
• Collaborate with physicians, nurse practitioners, and interdisciplinary teams to create care plans.
• Address inquiries from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and implement them as part of care plans.

• Salary Range: $90,000 - $115,000 Annual Salary
