Job summary

Pathways Healthcare is seeking a skilled Home Health Nurse to provide in-home patient care across Boston and surrounding areas. This per diem role involves assessment, planning, and execution of individualized care plans, with coordination with physicians and interdisciplinary teams.

The ideal candidate holds an active nursing license, a WCC certification, and 2+ years of geriatric nursing experience, plus a valid driver’s license and CPR certification.

Qualifications

• Active Nursing license in the state(s) where the position is based.
• Active Wound Care Certification (WCC).
• Preferably 2+ years of nursing experience with geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Valid driver's license and own a reliable insured vehicle.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed.
• Assess patients and families to determine home care needs.
• Perform physical assessments and obtain medical histories.
• Develop and revise plans of care based on patient status.
• Collaborate with physicians, NPs, and teams to create care plans.
• Address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and implement them.

Responsibilitie** s:

• Initiate appropriate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
• Responsively address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.
