Job summary

Pathways Healthcare is seeking a skilled home health nurse to provide direct care in patients' homes on a per diem basis. The role covers Worcester, MA and surrounding areas with a flexible schedule and per visit rates.

The ideal candidate holds an active nursing license in the state and a wound care certification, with 2+ years in nursing and geriatrics preferred. A valid driver’s license and reliable vehicle are required.

Qualifications

• Must hold an active Nursing license in the state where the position is based.
• Active Wound Care Certification (WCC) required.
• Preferably 2+ years of nursing experience, geriatrics preferred.
• Nursing degree from an accredited institution.
• Maintains current BLS CPR certification.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed.
• Assess patients and families to determine home care needs.
• Perform thorough physical assessments and collect medical histories.
• Develop and revise plans of care based on patient status.
• Collaborate with physicians and teams to create Plans of Care.
• Address inquiries from patients, caregivers, and medical teams.
• Provide counseling and support before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and implement them.

Responsibilitie s:

• Initiate appropriate preventive and rehabilitative nursing procedures.

• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
• Responsively address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.

Qualifications:

• Must hold an active Nursing license in the State where the position is based.
• Must hold an active Wound Care Certification (WCC).
• Preferably 2+ years of nursing experience, with a focus on geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver’s license and own a reliable, insured vehicle.
