Job summary

Pathways Healthcare seeks a Per Diem Home Health Nurse to assess, plan, and deliver patient-centered care in home settings around Concord. You will work with physicians, nurses, and caregivers to coordinate robust care plans and ensure smooth transitions during discharge planning.

Requirements include an active nursing license, WCC certification, and BLS CPR. A nursing degree and at least 2 years in geriatrics are preferred, with a valid driver’s license and dependable vehicle for visits.

Qualifications

• Active nursing license in the state(s) where the position is based.
• Active Wound Care Certification (WCC).
• Preferably 2+ years of nursing experience, geriatrics focus.
• Nursing degree from an accredited institution.
• Excellent bedside manners and communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care.
• Address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.
