Job summary

Adaptive Home Health is seeking an RN Case Manager to deliver high-acuity home health care in the field. You will own patient assessments, wound care and infusion therapies, create and maintain a plan of care, and advocate for patients and families.

This full-time role offers autonomy, collaboration with a multidisciplinary team, and opportunities to leverage technology to reduce documentation burden and improve patient outcomes. Mileage is provided in addition to productivity compensation.

Qualifications

• Must hold Texas RN license; 3+ years clinical nursing experience; comfortable with wound care and infusions; CPR (BLS) required; field-based work with Texas driver’s license; strong time management and communication.

Responsibilities

• Own patient assessments and documentation including OASIS.
• Deliver high-acuity home health care with wound care and infusion therapy.
• Maintain a clear plan of care through communication with physicians, care team, patients and families.
• Advocate for patients and ensure continuity of care across disciplines.
• Drive case management with care coordination, follow-ups, and barrier resolution.
