Job summary

Northfield Hospital + Clinics is seeking an RN Case Manager to provide direct professional nursing care for medical/surgical patients in the Wound Healing Center. The role emphasizes critical thinking, assessment, planning and evaluation of patient care, with a focus on safe environments for patients and families.

You will implement nursing interventions, monitor patient progress, and educate families per diagnosis and treatment plans.

Qualifications

• Current RN license with Minnesota Board of Nursing in good standing.
• Current BLS enrolled in NH+C RQI program and complete within 90 days.
• Crisis Prevention Certification (CPI) within six months.
• Workplace violence education in New Employee Orientation and annually.
• Trauma training within 1 year of hire.
• EKG rhythm interpretation competence documented within 6 months.
• Excellent verbal and written English communication.

Responsibilities

• Provide direct professional nursing care for medical/surgical patients.
• Assess, plan, implement and evaluate patient care using the nursing process.
• Deliver age-specific care to neonate, adolescent and adult patients.
• Direct and delegate to support personnel in implementing the plan of care.
• Administer medications and manage IV fluids and blood products as needed.
• Document patient response and educate patients and families.
