Healing Partners is seeking a Wound Care Nurse Practitioner to plan, direct and evaluate health care across long-term care facilities. This full-time role requires daily travel to multiple SNFs in the local area, with a Monday–Friday day schedule and no nights or on-call.

You will diagnose and treat wound-related illnesses, prescribe, and coordinate care with physicians and the medical director. Qualifications include an active RN and APRN license, a Master’s in Nursing, and a Covid vaccination

Qualificações

• Proof of Covid vaccination.
• Licensed as a registered nurse in the state.
• Active APRN license.
• Master’s degree of Nursing.
• Board approval to perform medical acts.
• Excellent organizational skills.
• Strong analytical and problem-solving skills.

Responsabilidades

• Plan, direct and evaluate resident and employee health care in long-term care.
• Daily travel to various facilities. Regions can vary based on scheduling needs.
• Examine and treat chronic and acute wound-related illnesses and accidents.
• Interpret diagnostic tests and recommend follow-up.
• Refer to and update physicians, write prescriptions, and perform bedside procedures for wound care.
• Work with the medical director in the care and treatment of residents and staff.
• Perform debridement of wounds as needed.
• Manage topical wound care and wound management products.
• Recommend associated services to improve wound healing outcomes.
• Educate patients and Primary Care Physicians on current wound management practices.
