AlignMed Partners seeks a wound care Advanced Practice Provider (NP/PA) for a 16–40 hour per week float role across northern New Hampshire centers. The position offers a base salary range of $112,320–$137,280/year with a no-cap incentive program.

The ideal candidate has at least 1+ year wound care APP experience and holds NP or PA licensure with appropriate certifications. NH licensure, DEA, and state-specific credentials required.

Formación

• Graduate from an accredited nursing program and licensed as a registered nurse.
• Certified as a Nurse Practitioner with a Master’s in Nursing; concentration in Older Adult and/or Family NP.
• PA must graduate from ARC-PA accredited program and hold PA licensure and NCCPA certification.
• Wound certification (CWS, WOCN) or actively pursuing with APP wound experience.
• Minimum two years of wound care experience with adult/geriatric populations.

Responsabilidades

• Provide acute and chronic wound care on-site in nursing centers, collaborating with other providers.
• Coordinate with Center Leadership to align with center goals and improve patient outcomes.
• Work with Medical Director/Attending Physician for patient care decisions.
• Participate in delivery of high-quality health care with compassion.
