AlignMed Partners is seeking an Advanced Practice Wound Provider to deliver acute and chronic wound care across northern New Hampshire centers. The role offers flexible hours from part-time to full-time (16–40 hours weekly) and a base salary range with incentive bonus.

The Advanced Practice Wound Provider collaborates with Center Leadership and medical staff to optimize patient care and outcomes while supporting the organization’s mission and values.

Qualifications

• Graduate from an accredited School of Nursing and licensed RN.
• APN with Older Adult and/or Family NP focus; PA path considered with license and DEA.
• Wound certification (CWS/WOCN) or actively pursuing wound certification.
• Minimum 2 years wound care experience working with adult/geriatric population.

Responsabilités

• Provide acute and chronic wound care services to nursing center residents.
• Collaborate on-site with Center Leadership for goals and priorities.
• Coordinate with Medical Director/Attending Physician for patient care.
• Support center mission of high-quality health care and compassion.
