Job summary

Growing Mobile Wound Care Practice is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to provide mobile wound care across Northern New Hampshire, including Berlin. Part-time, 2-3 days per week, with an extremely flexible schedule and compensation of $90–110/hour.

No prior wound care experience required, but training and ongoing clinical support are provided. Responsibilities include treating chronic and complex wounds in SNFs, LTCs and home care, and developing individualized care

Qualifications

• Board Certified Nurse Practitioner or Physician Assistant
• 1+ year of APP experience
• Wound care or Urgent Care experience is a plus, but not required
• Comfortable working independently while collaborating with Medical Directors and the broader clinical team

Responsibilities

• Provide mobile wound care to patients in SNF, LTC and home care settings in Coos County, NH
• Develop individualized treatment plans for each patient
• Utilize advanced wound care modalities and evidence-based treatments
• Maintain an independent, collaborative workflow with Medical Directors and the clinical team

• Collaborative team of 4 APPs and 2 Medical Directors
• Provides specialized wound care to patients in SNF, LTC and home care settings
• Focused on treating complex and chronic wounds and helping patients avoid unnecessary hospitalizations
• Providers are supported with comprehensive wound care training
• Uses advanced wound care modalities, including UltraMIST, along with other evidence-based treatment options

About the Position:

Provide mobile wound care to patients throughout Coos County. Treat chronic and complex wounds in Skilled Nursing Facilities, Long-term Care and some home care. Part‑time position, 2‑3 days/week, Extremely flexible scheduling. Utilise advanced wound care modalities and develop individualized treatment plans for each patient. No prior wound care experience required - wound care training and ongoing clinical support.
