Job summary
The Chateau at Brooklyn Rehabilitation and Nursing Center in Brooklyn, NY is seeking a compassionate Wound Care LPN to join our skilled nursing team. You will provide specialized wound care, support prevention of pressure injuries, and help residents heal.
The ideal candidate holds a NY LPN license, has wound-care experience, and demonstrates strong assessment, documentation, and communication skills. This full-time role offers daytime hours with occasional weekends and a comprehensive benefits
Qualifications
- Current New York Licensed Practical Nurse (LPN) license.
- Wound care experience in skilled nursing or long-term care preferred.
- Wound care certification is a plus.
- Strong clinical assessment and documentation skills.
- Excellent communication and organizational abilities.
Responsibilities
- Perform wound assessments and provide treatments per physician orders and facility protocols.
- Monitor and document wound progress, communicate changes to the care team.
- Develop and implement individualized wound care plans.
- Ensure accurate and timely documentation of treatments and assessments.
- Educate residents, families, and staff on wound prevention and treatment.
- Participate in skin rounds and quality improvement initiatives.
- Maintain compliance with all regulations and standards.
- Collaborate with physicians, NPs, and nursing staff to achieve resident outcomes.
