Job summary

The Chateau at Brooklyn Rehabilitation and Nursing Center in Brooklyn, NY is seeking a compassionate Wound Care LPN to join our residential care team.

This full-time role focuses on wound assessment, treatment per physician orders, and collaboration with the interdisciplinary team to drive resident healing and comfort. LPN license in NY required; wound care experience preferred and certification a plus.

Qualifications

• Current and active New York Licensed Practical Nurse (LPN) license.
• Wound care experience in SK nursing facility or long-term care preferred.
• Wound Care Certification is a plus.
• Strong clinical assessment and documentation skills.
• Excellent communication and organizational abilities.
• Passion for delivering exceptional resident-centered care.

Responsibilities

• Perform wound assessments and provide treatment per physician orders and facility protocols.
• Monitor and document wound progress and communicate changes to the interdisciplinary team.
• Develop and implement individualized wound care plans.
• Ensure accurate and timely documentation of all treatments and assessments.
• Educate residents, families, and staff on wound prevention and treatment strategies.
• Participate in skin rounds and quality improvement initiatives.
• Maintain compliance with all federal, state, and facility regulations and standards.
• Collaborate with physicians, nurse practitioners, and nursing staff to ensure resident outcomes.
