The Chateau at Brooklyn Rehabilitation and Nursing Center in Brooklyn, NY is seeking a compassionate and experienced Wound Care LPN to join our skilled nursing team. This role focuses on wound management and delivering resident-centered care to our residents.

The Wound Care LPN will perform wound assessments, develop and implement care plans, document progress, and educate residents and families. Collaboration with physicians and nursing staff ensures optimal healing outcomes.

Qualifications

• Current and active New York LPN license.
• Wound care experience in a skilled nursing facility or long-term care setting preferred.
• Wound Care Certification is a plus.
• Strong clinical assessment and documentation skills.
• Excellent communication and organizational abilities.
• Passion for delivering exceptional resident-centered care.

Responsibilities

• Perform wound assessments and provide treatment per physician orders and protocols.
• Monitor and document wound progress; communicate changes to the team.
• Develop and implement individualized wound care plans.
• Ensure accurate and timely documentation of treatments and assessments.
• Educate residents, families, and staff on wound prevention and treatment strategies.
• Participate in skin rounds and quality improvement initiatives.
• Maintain compliance with regulations and standards.
• Collaborate with physicians and nursing staff to achieve excellent resident outcomes.
