Qualifications

• Current and active New York Licensed Practical Nurse (LPN) license.
• Experience in wound care in skilled nursing or long-term care setting preferred.
• Wound care certification is a plus.
• Strong clinical assessment and documentation skills.

• Passion for delivering resident-centered care.

Responsibilities

• Perform wound assessments and treatments per physician orders and facility protocols.
• Monitor wound progress and document changes for interdisciplinary team.
• Develop and implement individualized wound care plans.
• Ensure accurate and timely documentation of treatments and assessments.
• Educate residents, families, and staff on prevention and treatment strategies.
• Participate in skin rounds and quality improvement initiatives.
• Maintain compliance with federal, state, and facility regulations.
• Collaborate with physicians and nursing staff to achieve resident outcomes.
