Job summary
The Chateau at Brooklyn Rehabilitation and Nursing Center is seeking a compassionate Wound Care LPN to join our skilled nursing team in Brooklyn, NY. You will provide specialized wound care, prevent and treat pressure injuries, and help residents achieve optimal healing outcomes.

Qualifications
- Current New York LPN license required.
- Wound care experience in skilled nursing or long-term care preferred.
- Strong clinical assessment and documentation skills.
- Excellent communication and organizational abilities.
Responsibilities
- Perform wound assessments and provide treatment per physician orders and facility protocols.
- Monitor and document wound progress and communicate changes to the interdisicplinary team.
- Develop and implement individualized wound care plans.
- Ensure accurate and timely documentation of all treatments and assessments.
- Educate residents, families, and staff on wound prevention and treatment strategies.
- Participate in skin rounds and quality improvement initiatives.
- Maintain compliance with all regulations and standards.
- Collaborate with physicians, NPs, and nursing staff to ensure resident outcomes.
