Job summary

The Chateau at Brooklyn Rehabilitation and Nursing Center in Brooklyn, NY is seeking a compassionate Wound Care LPN to join our skilled nursing team. You will provide specialized wound care, monitor progress, and help develop individualized care plans on site.

Qualified candidates hold an active NY LPN license, with prior wound care experience in long-term care. Certification in wound care is a plus, and strong documentation skills are essential for resident-centered care.

Qualifications

• Current and active New York Licensed Practical Nurse (LPN) license.
• Previous wound care experience in a skilled nursing facility or long-term care setting preferred.
• Wound Care Certification is a plus.
• Strong clinical assessment and documentation skills.
• Excellent communication and organizational abilities.
• Passion for delivering exceptional resident-centered care.

Responsibilities

• Perform wound assessments and provide treatment in accordance with physician orders and facility protocols.
• Monitor and document wound progress and communicate changes to the interdisciplinary team.
• Assist in developing and implementing individualized wound care plans.
• Ensure accurate and timely documentation of all treatments and assessments.
• Educate residents, families, and staff on wound prevention and treatment strategies.
• Participate in skin rounds and quality improvement initiatives.
• Maintain compliance with all federal, state, and facility regulations and standards.
• Collaborate with physicians, nurse practitioners, and nursing staff to ensure excellent resident outcomes.
