Job summary

Healogics, LLC. is seeking a Wound Care Center Licensed Practical Nurse (LPN) to provide wound-related patient care under the direction of an RN or Provider.

You will assist with vein clinic activities after training, manage intakes/discharges, and support case management where allowed by state regulations. Qualifications

• LPN/LVN license current in state of practice.
• Graduate of an approved practical nursing program.
• Minimum 2 years' LPN/LVN experience, including SNF or LTACH preferred.
• Wound care experience preferred.

Responsibilities

• Provide wound-related patient care under RN/Provider.
• Assist in vein clinic after training.
• Assist with intake and discharge.
• May support case management per state regulations.
• Gather patient information and equipment.
• Report quality issues to Clinical Nurse Manager.
• Perform hyperbaric therapy-related duties after training.
• Communicate with team to ensure quality of care.
• Other duties as assigned.
