Job summary

Healogics is seeking a Wound Care Center Licensed Practical Nurse (LPN) to provide wound-related patient care under RN or Provider supervision in a fast-paced center. You will assist in vein clinic, help with intake/discharge, and support case management as required.

Strong BLS and nursing systems knowledge are essential. Ideal candidates hold an LPN/LVN license and have 2+ years of LPN/LVN experience, with wound care exposure preferred.

Qualifications

• High School Diploma or GED.
• Graduate from an accredited practical nursing program/state approved educational program
• Current LPN/LVN license in the state of practice
• Two (2) or more years’ experience as LPN/LVN, to include one (1) or more years in SNF or LTACH setting
• Wound care experience preferred

Responsibilities

• Provide wound-related patient care under RN/provider guidance
• Assist in vein clinic after training
• Assist with intake and discharge
• Case management assistance under RN supervision
• Gathers patient information
• Gathers equipment for RN/provider
• Reports quality of care issues to Clinical Nurse Manager
• Communicate with team about therapies to ensure quality care
• Perform hyperbaric therapy related duties after training
• Documentation assistance as needed
• Perform other duties as delegated
