Zusammenfassung

Extended Care in Des Plaines, IL seeks a dedicated Wound Care Coordinator to oversee wound care management and skin integrity for our residents. The RN or LPN will provide high-quality wound care, implement preventative measures, and ensure compliance with federal and state regulations and facility policies.

Qualifikationen

• Active Nursing License as required by the state.
• Current CPR certification.
• Minimum two (2) years of experience as an RN or LPN.
• Ability to lift and move patients and items up to 75 lbs.
• Strong verbal and written communication skills in English.

Aufgaben

• Utilize the nursing process for assessment, planning, implementation, and evaluation of wound and skin care.
• Conduct regular wound care rounds, ensuring timely documentation and tracking of resident skin conditions.
• Assess resident skin integrity and document medical history related to wound care.
• Implement preventative wound care strategies in resident care plans.
• Prioritize and execute nursing interventions for wound management.
• Communicate with physicians and interdisciplinary teams regarding wound treatment needs.
• Initiate nutritional interventions to support wound healing.
• Perform direct resident care in compliance with facility policies and procedures.
• Ensure residents with incontinence receive appropriate pericare and preventative treatments.
• Maintain a safe and clean work environment, adhering to infection control protocols.
