Job summary

Zahav Healthcare Consulting is seeking a dedicated Wound Care Coordinator to oversee wound care management and skin integrity programs for residents in an Extended Care affiliated facility. The RN or LPN will provide high‑quality wound care, implement preventative measures, and ensure compliance with federal and state regulations and facility policies.

Qualifications

• Active Nursing License (state required)
• Current CPR certification
• Minimum 2 years of RN or LPN experience
• Ability to lift/move patients and items up to 75 lbs
• Strong vision abilities (near/far/peripheral/depth)
• Excellent verbal and written communication in English
• Ability to cope with mental and emotional stress
• Strong attention to detail and prioritization of wound care needs

Responsibilities

• Utilize the nursing process for assessment, planning, implementation, and evaluation of wound and skin care
• Conduct regular wound care rounds with timely documentation
• Assess resident skin integrity and document medical history related to wound care
• Implement preventative wound care strategies in care plans
• Prioritize nursing interventions for wound management
• Coordinate with physicians and interdisciplinary teams on wound treatment needs
• Initiate nutritional interventions to support wound healing
• Provide direct resident care in line with facility policies
• Ensure pericare and preventative treatments for residents with incontinence
• Maintain a safe, clean work environment with infection control adherence
