Job summary

Lemont Nursing and Rehabilitation Center in Illinois is seeking a Wound Care Coordinator to oversee resident wound and skin care. Active RN/LPN license, CPR certification, and at least two years of wound care experience are required; you will perform assessments, care planning, and coordinate with physicians and the interdisciplinary team.

Qualifications

• Active Nursing License (RN or LPN) as required by state
• Current CPR certification
• Minimum two years of wound care experience
• Ability to lift/move patients up to 75 lbs
• Strong communication and documentation skills in English
• Attention to detail and prioritization of wound care needs

Responsibilities

• Utilize nursing process for assessment, planning, implementation and evaluation of wound care
• Conduct wound care rounds and document resident skin conditions
• Assess resident skin integrity and review medical history related to wounds
• Implement preventive wound care strategies in care plans
• Perform nursing interventions for wound management
• Coordinate with physicians and interdisciplinary teams on treatment needs
• Initiate nutritional interventions to support wound healing
• Provide direct resident care following facility policies and infection control protocols
• Ensure pericare and prevention for incontinence in residents
• Maintain a safe, clean work environment adhering to infection control protocols
