Job summary

Prairie Oasis in South Holland, Illinois is seeking a compassionate Wound Care Nurse to deliver high-quality wound management for our residents. You will perform debridement, dressings, assessments, and collaborate with the care team to prevent re-hospitalization.

The role requires an active IL LPN or RN license, at least 1 year in wound care, strong knowledge of wound care principles, and excellent communication.

Qualifications

• Current licensure as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in Illinois.
• Minimum 1 year of experience as a Wound Care Nurse.
• Strong knowledge of wound care principles, practices and best methods.
• Excellent communication and interpersonal skills.
• Ability to work effectively in a fast-paced environment.

Responsibilities

• Provide wound care management and treatment to residents, including debridement, dressing changes, and wound assessment.
• Conduct comprehensive wound assessments to identify areas for improvement and develop individualized treatment plans.
• Collaborate with other healthcare professionals to ensure seamless care coordination and effective communication.
• Monitor and document resident progress, updating treatment plans as necessary to ensure optimal healing and prevent complications.
• Educate residents, families, and healthcare teams on wound care best practices and prevention strategies.
• Participate in multidisciplinary team meetings to discuss resident care and develop comprehensive plans to achieve desired outcomes.
