Job summary

SVdP is seeking a Workforce Case Manager in Phoenix to serve guests experiencing homelessness. You will map out service plans, assist with employment and housing opportunities, and connect participants with resources across shelters and community programs.

The role requires strong communication, organization, and trauma-informed skills. You will collaborate with staff, Fellows, and volunteers to promote stability and progress for program participants.

Qualifications

• Bachelor’s degree in social work or related field preferred.
• High School diploma or GED required.
• 1+ years experience in Case Management or 2+ years in human services.

Responsibilities

• Carry a caseload of workforce participants, meeting with each resident at least weekly.
• Actively recruit new participants and assess needs for workforce programs.
• Complete assessments and individualized service plans to identify needs, strengths, goals, and barriers.
• Monitor and document resident progress toward stabilization, income, and housing goals.
• Maintain timely records in data systems within 24 hours of interaction.
• Coordinate incident intervention and conflict resolution with staff and security.
• Connect clients with resources such as skills training, education, and certifications.
• Coordinate with employers, training providers, and community organizations for job placement.
• Attend staff meetings, case conferencing, trainings, and agency events.
• Support volunteers and volunteer engagement.
• Other duties as assigned by program management.
