• Monitors patient conditions, adjusts care plans, mobilizes resources, and collaborates with the care team to influence outcomes.
• Upholds a culture of safety and ensures accurate, timely documentation in EHR.
• May float to other units within the service area as needed.
• Provide patient and family education and support throughout the continuum of care.
