Job summary

Access Services is seeking a Case Manager of Justice Related Services to support adults in Montgomery County, with an office base in Plymouth Meeting, PA. You will guide clients through legal and mental health systems, helping with housing, employment, and personal wellness while advocating for success.

The role requires a Bachelor’s degree in a related field and at least 2 years of case management experience, with driving throughout the county.

Qualifications

• Bachelor’s degree in a related field such as psychology, social work, criminal justice, etc. is required.
• 2+ years of case management experience with adults with serious mental illness preferred.
• Willingness to drive throughout Montgomery County to meet clients is required.

Responsibilities

• Engage adults with serious mental illness to facilitate support and linkage to resources for successful reentry (housing, career, community involvement).
• Meet with clients in jail, in the community, at court, in hospitals, and with law enforcement.
• Organize and coordinate services for adults with serious mental illness in the forensic system using Blended Case Management guidelines.
• Ensure care aligns with Access Services philosophy and meets individual needs.
• Build collaboration through community outreach and awareness.
• Complete case notes and documentation for the program.
• Attend team meetings and case reviews.
• Attend county, committee, and consumer meetings as needed.

Education
