Job summary

Cornerstone Family Healthcare is seeking a WHIN Peer Navigator to deliver peer-driven services in Rockland, Dutchess, Orange, and Westchester counties. Based in the Newburgh office, this role requires some evenings and weekends, up to 14 hours per week, under supervision of the Program Supervisor.

The position focuses on increasing awareness of HIV/HCV/STI status, addressing SDOH through workshops and outreach, and linking young women of color at high risk to testing and treatment services.

Qualifications

• High School Diploma or GED with lived shared experience of priority population.
• One year of experience in peer education, HIV/HCV/STI counseling and/or disease prevention.
• Knowledge of HIV/HCV/STI’s and harm reduction, mental health or community resources.

Responsibilities

• Outreach to recruit and develop a core group of young WoC to act as peers for the program.
• Conduct outreach to community partners and maintain collaborative relations with service providers.
• Lead groups and events that provide health education and risk reduction strategies.
• Provide referrals for testing, linkage to treatment, SDOH, and other services as needed.
• Enter data into the AIRS system for each intervention and service.
