Job summary

Qualifications

• High School Diploma with experience in peer education, HCV counseling and/or disease prevention.
• Familiarity with the target population and geographic area assigned.
• Commitment to undergo training about HIV/HCV/STI, harm reduction, mental health and community resources.

Responsibilities

• Observe HIPAA and confidentiality standards and assist program staff in promoting services.
• Provide referrals and follow-up for testing, linkage to care and other services as needed.
• Conduct outreach through social media, presentations and community networking to reach high-risk individuals.
• Make reminder calls for appointments, provide transportation support and other client services.
• Attend required department and staff meetings and maintain program documentation.
