Job summary

Westchester Medical Center seeks a Peer Coordinator to provide client-centered support and education to clients with HIV. You will coordinate services with the care team, advocate for positive health outcomes, and help patients navigate healthcare systems in a compassionate, confidential setting.

Role requires experience living with HIV, strong communication, and commitment to education and adherence support. Training and ongoing certification are provided, with bilingual skills preferred.

Qualifications

• Must live with HIV and manage care and medication adherence.
• Experience in sexual health education and harm reduction preferred.
• Strong communication with patients and within community.

Responsibilities

• Identify and address barriers to care; promote self-efficacy through teaching and coaching.
• Orient new patients to grant and clinic services; introduce patients to staff.
• Navigate patients to medical visits and other appointments.
• Outreach new referrals within 5 days; make reminder calls and follow-ups.
• Provide educational materials and support education/adherence through group activities.
• Meet patients in their home and escort to appointments as needed.
• Documentation of interventions in AIRS; maintain logs and timely data submission.
• Participate in the Peer Certification program and maintain confidentiality.
• Work as a team to support program goals and participate in QA activities.
