Cornerstone Family Healthcare in Newburgh, NY is hiring a Prevention Specialist for a mobile van-based outreach program. You will deliver syringe support services, harm reduction education, and counseling while referring clients to comprehensive health services in the Mid-Hudson region.

The role supports two mobile units across Dutchess, Ulster, Orange, and Sullivan Counties and the Newburgh Hub, requiring strong community partnerships and data entry accuracy.

Qualifications

• Associate’s Degree with at least two years of experience working with PWUD/PWID outreach, including knowledge of harm reduction, behavioral health, social services, and community resources.
• Minimum of one year experience working with PWUD, drug treatment services, knowledge of HIV/HCV/STI, harm reduction, healthcare, and community resources.
• Experience working with PWUD/PWID population.
• Bilingual (English/Spanish) preferred.

Responsabilités

• Observe and abide by HIPAA and the HIV Confidentiality Law.
• Maintain ongoing relationships with community-based resources, providers, and local police precincts in the area in which the program provides services.
• Attend and provide training for the Opioid Overdose Prevention Program (NARCAN).
• Provide referrals for HIV/STI/HCV testing and counseling, PrEP/PEP, drug treatment, medical, behavioral health, social services, as well as other supportive services.
• Ensure that client charts are complete and filled out properly in accordance with program standards in keeping with the objectives as stated in the annual workplan and in compliance with agency and program policies and procedures.
• Enter outreach and Narcan trainings into the database (AIDS Institute Report System - AIRS) consistent with program requirements. Will also assist with SSP transaction data and intakes as needed.
• Review, develop and continually assess the need for services in the community where SSP provides coverage, to ensure services are effective and reaching the priority population.
• Actively participate in weekly supervision and monthly Harm Reduction team meetings.
• Assist in program coverage when required including street outreach, special arrangements, and mobile unit.
• Attend and participate in monthly department, All Staff and other required meetings.
• Be familiar with Cornerstone policies and procedures and the Employee Handbook.
• Maintain confidentiality of all aspects of Cornerstone including, but not limited to, patient confidentiality, financials, and employee relations.
• Provide task supervision to the program peer.
• Perform other related duties as assigned.
