Job summary

Griffin Health in Derby, CT is seeking a Wellness Facilitator/Intake Coordinator to assess patients for admission into Wellness programs, assist in exercise prescriptions, and support data reporting for employer groups. This role reports to the Employer Wellness Director and requires strong Excel/data skills and a patient-centered approach.

You will promote program options, coordinate with clinical staff, manage wellness files, and help patients engage in lasting lifestyle changes while working

Qualifications

• Bachelor's degree or higher in Allied Health, Exercise Science or related field.
• Clear written and verbal communication skills.
• Willingness to learn and grow with a small, fast-growing team.
• Ability to work in cross-functional teams.
• Proficiency with MS Office: Excel, PowerPoint, CSV.
• Minimum of one year in the wellness space.

Responsibilities

• Work in collaboration with clinical staff to ensure the smooth scheduling and transition of patients to the wellness intake process
• Have regular and positive communication with clinical staff to promote scheduling of patients for wellness programs
• Have a mastery of system program offerings
• Ability to promote programs in conversation with patients
• Manage employer group files associated with our wellness platform
