Job summary

Cascadia Health seeks a Peer Provider for Wellness to join an interdisciplinary team in a trauma-informed, recovery‑oriented setting in the Portland metro area. You will lead health-promotion groups and provide one-on-one support aligned with clients’ goals and service plans.

You must identify with lived behavioral health experience, and participate in training, data collection, and collaborative treatment planning to support client well‑being and housing goals.

Qualifications

• Must have completed Peer Support training approved by the Oregon Health Authority (OHA).
• Must have completed a Peer Wellness training approved by the Oregon Health Authority (OHA).
• A Traditional Healthcare Worker (THW) certification is required to stay current with OARs.
• Ability to travel to multiple locations throughout the Portland Metro area.
• BLS certification within 60 days of hire; training provided if needed.

Responsibilities

• Identify as a person with lived mental health service experience.
• Serve as a recovery-focused role model and share your recovery story when appropriate.
• ProvideTherapeutic interventions aligned with mental health treatment plans and healthcare objectives.
• Lead health-promotion groups and individual wellness activities.
• Administer outcome measures and support data collection for grants.
• Help reduce emergency department visits by following up with clients after visits.
• Attend supervision, meetings, and case conferences.
• Maintain electronic health records per agency standards and HIPAA.
