Job summary

The Peer Provider at Cascadia Health is based at the Plaza Health Center in SE Portland, OR, working Monday through Friday, 8:30 a.m. to 5:00 p.m.

You will deliver peer support in clinics, the community, and individual homes as part of Integrated Team Based Care. As a lived-experience professional, you will collaborate with the treatment team, promote trauma-informed, person-first approaches, and help individuals access housing, health services, and community resources while maintaining

Qualifications

• THW Peer Support Certification required.
• Experience as a former or current recipient of behavioral health services and self-disclosure is required.

Responsibilities

• Model empowerment, self-determination, and self-advocacy; share recovery experiences as appropriate.
• Engage with individuals using strengths-based approaches and harm reduction.
• Coordinate with care teams to ensure continuity of services and resource development.
• Provide peer support in the program, community, and homes as needed.
• Assist with housing, benefits access, and connections to community resources.
• Facilitate therapeutic groups and participate in supervision and case conferences.
