Job summary

Cascadia Health is seeking a Peer Provider to join the Wellness program across three Portland-area health centers. The role supports treatment in an integrated care setting, emphasizing trauma-informed approaches and recovery-oriented practice.

You will lead health promotion groups, provide individual support, and help clients pursue goal-based wellness plans aligned with service plans. Travel between sites is required, with standard 40-hour weeks, Mon-Fri, 8:30 a.m. - 5:00 p.m.

Qualifications

• Must have completed Peer Support training approved by the Oregon Health Authority (OHA) or Peer Wellness training approved by the OHA.
• A Traditional Healthcare Worker (THW) certification is required to stay compliant with the OARs.

Responsibilities

• Provide therapeutic interventions aligned with mental health treatment plans and integrated healthcare objectives.
• Lead health promotion groups and one-on-one wellness activities.
• Document services in electronic health records per regulations and program protocols.
• Support clients and caregivers through recovery-focused, trauma-informed care approaches.
• Travel to multiple locations throughout the Portland metro area for service delivery.
