Job summary

Brigham House is seeking a Wellness Nurse RN-LPN for Windrose at Woburn Assisted Living Memory Care in Woburn, MA. This full-time position supports the Resident Care Director in delivering and coordinating resident care services and assisting with the medication management program.

The ideal candidate is a licensed RN, LPN, or LVN in good standing, with the ability to work varied shifts, interact with residents and families, and participate in training and staff orientation.

Qualifications

• Currently licensed RN/LPN/LVN in good standing.
• Certification or state licensing per guidelines.
• Must complete Hallkeen Community training programs.
• Able to follow detailed but standard instructions.
• Able to work various schedules and shifts.

Responsibilities

• Assist in supervising the medication program and resident self-administration.
• Assist with medication deliveries and med box audits.
• Support ADLs and independence in activities of daily living.
• Maintain positive relations with residents, families and physicians.
• Assist in assessments, service plans and family communications.
• Assist in training assisted living staff and orient new staff.
• Attend in-service programs and staff meetings.
• Assist with scheduling Resident Care Assistants.
• RN will maintain GAFC.
• May perform other duties as assigned.
