Job summary

The Briarwood Community in Worcester, MA is seeking a compassionate Wellness Nurse (RN or LPN) to support Independent Living residents with health assessments, guidance, advocacy, and wellness education.

This role serves as a key resource for residents navigating healthcare needs, promoting independence, safety, and well-being. Positions are available as part-time or full-time with a collaborative, resident-centered environment and comprehensive benefits.

Qualifications

• Current Massachusetts RN or LPN license in good standing.
• Current Basic Life Support (BLS) Certification.
• Minimum of two years of experience in Skilled Nursing, Assisted Living, Memory Care, or Independent Living.
• Experience working directly with residents, families, healthcare providers, and community service organizations.
• Strong assessment, communication, and organizational skills.
• Proficiency with documentation and basic office technology.

Responsibilities

• Conduct resident wellness assessments to help determine appropriate medical care and services.
• Provide guidance and direction regarding healthcare resources, providers, and treatment options.
• Assist residents in navigating healthcare systems, appointments, and medical processes.
• Maintain accurate and timely documentation of resident health concerns and wellness issues.
• Provide professional verbal and written communication with residents, families, healthcare providers, and team members.
• Participate in weekly Risk/Tracking meetings and other interdisciplinary meetings as needed.
• Meet with prospective Independent Living residents and provide wellness assessments during the admission process.
• Collaborate with community agencies, educational institutions, and service providers to develop wellness and educational programs.
• Support the Home Companion Program through assessments, recommendations, and ongoing guidance.
• Advocate for residents while promoting independence and informed decision-making.
