Schedule: Part Time, Weekend Days, Every other weekend commitment

Salary: Starting at $35.00 per hour

The Wellness Nurse supports the health and well-being of residents through ongoing clinical monitoring and support. The role involves providing assistance to community nurses, caregivers, and medication personnel to ensure regulatory compliance and clinical excellence. Flexibility, adaptability, and a warm, encouraging disposition are essential for working effectively as part of a team.

• Assist in coordinating the resident admission process, including screening, assessment, and care needs
• Ensure care is delivered as indicated in resident care plans
• Administer medications and treatments, ensuring they are given per physicians’ orders
• Coordinate care and services with outside providers, including resident physicians
• Document and report care provided and resident observations, noting any unusual or significant changes
• Maintain knowledge of state regulations, policies, and procedures

Qualifications:

To succeed in this position, the following experiences, strengths, and skills are preferred:

• Knowledge of state regulations, policies, and procedures related to compliance
• Graduate from an accredited school of practical nursing
• Current nursing license and state registration (LPN or RN)
• Three years’ experience in assessment and geriatric nursing (preferred)
• Compassion, eagerness to learn, and a positive attitude towards residents
