Job Description

Schedule: Part Time, Weekend Days, Every other weekend commitment

Salary: Starting at $35.00 per hour

The Wellness Nurse supports residents' health and well-being through clinical monitoring and support, assisting community nurses, caregivers, and medication personnel to ensure compliance and excellence. Flexibility, teamwork, and a warm disposition are essential.

• Assist in resident admission processes including screening, assessment, and care needs
• Ensure care delivery per resident care plans
• Administer medications and treatments as per physician orders
• Coordinate care with outside providers, including resident physicians
• Document care provided and observe resident changes
• Maintain knowledge of state regulations, policies, and procedures

Qualifications

• Knowledge of state regulations, policies, and procedures for compliance
• Graduate from an accredited practical nursing program
• Current nursing license and registration (LPN or RN)
• Preferred: Three years’ experience in assessment and geriatric nursing
• Compassion, eagerness to learn, and a positive attitude
