Job summary

Sunrise Senior Living is seeking a Wellness Nurse (LPN/LVN) to support resident health, monitor care needs, and ensure safe medication practices. The role emphasizes clinical quality and regulatory compliance within a senior living environment.

You will conduct wellness assessments, assist with medication management, and collaborate with families, physicians, and the care team to provide compassionate, high-quality care.

Qualifications

• Active LPN/LVN license or license-eligible state certification.
• Minimum 1 year of experience in a senior living or clinical setting.
• Strong assessment, documentation, and care planning abilities.

Responsibilities

• Conduct wellness assessments and monitor resident health changes.
• Support medication management and perform MAR/TAR audits.
• Communicate with residents, families, physicians and care teams.
• Monitor incidents, falls and follow-up care.
