Job summary

A leading assisted living facility in California is looking for a Wellness Nurse (LVN) to supervise caregivers and ensure residents receive the necessary care. The ideal candidate will have a current California LVN License and at least one year of experience in direct nursing care for elderly patients. This position offers a competitive wage of $34.75 – $37.00 per hour, along with full-time benefits including medical and retirement plans.

Qualifications

• Graduate of an accredited school of nursing with a current California LVN License.
• At least one year of experience in nursing care for elderly or chronically ill patients.
• Knowledge of Title 22 RCFE Regulations preferred.

The Wellness Nurse assists the Director and supervises the direct care of the residents being provided by the caregivers.

Schedule

Full‑Time: Sunday – Thursday 2:00 pm – 10:30 pm

Wage

$34.75 – $37.00 per hour based on experience

Job Description

• Check that each resident is receiving the care and services that meet their needs and that their environmental cleanliness is maintained.
• Supervise the Medication Technicians to ensure compliance with RCFE regulations and that documentation and medication assistance are completed. Report all required information to a Director before the end of each shift.
• Conduct regular in‑services and trainings for medication technicians and caregivers on relevant topics.
• Audit medication records on a regular basis.
• Assist the Director with contacting resident’s attending physicians for significant changes in condition, new admission orders, discharge orders, and all necessary changes in medication and treatment orders.
• Notify resident family members/surrogate decision makers of significant changes in condition, changes in medication and treatment orders, obtain informed consents, develop resident care plans, and document all physician, resident, family member/surrogate decision maker communication.
• Respond quickly, calmly, and professionally to medical emergencies.
• Perform other duties as assigned by Supervisor.

What You Will Bring

• Graduate of an accredited school of nursing. Current California Licensed Vocational Nurse License in good standing with the LVN Board of Nursing.
• At least one year of experience providing direct nursing care to the elderly and chronically ill patients, preferably in an Assisted Living facility.
• Knowledge of Title 22 RCFE Regulations preferred.
• Current First Aid and CPR certification.
