Job summary

The Aspenwood Company is seeking a compassionate Wellness Nurse to deliver high-quality care at Village on the Park Onion Creek in Austin. You will administer medications, oversee care delivery, and support a team focused on dignity, safety, and person-centered service.

Ideal candidates hold an LVN/LPN license, BLS, and experience in assisted living or long-term care. You will collaborate with physicians, families, and staff to ensure excellent resident outcomes and compliant, compassionate care.

Qualifications

• Current LVN/LPN license in the state of employment.
• BLS/CPR certification.
• Minimum 6 months to 1 year experience in Assisted Living or Long-Term Care.
• Proficient in Electronic Health Records (EHR) and basic computer skills.
• Strong verbal and written communication skills.
• Knowledge of HIPAA regulations and compliance standards.
• Ability to respond with patience and understanding to dementia-related behaviors.
• Physical ability to perform essential job functions with or without reasonable accommodation.

Responsibilities

• Administer medications safely and accurately per physician orders and regulatory standards.
• Oversee medication aides and conduct monthly medication reconciliation and audits.
• Coordinate daily staff assignments to ensure consistent resident care.
• Communicate effectively with physicians, families, and care teams regarding resident needs.
• Provide direct care including ADLs: bathing, dressing, grooming, toileting, mobility, and incontinence support.
• Respond to medical and behavioral emergencies with sound clinical judgment.
• Complete clinical documentation including assessments, evaluations, and service plans.
• Document incidents and communicate promptly with leadership.
• Monitor care team documentation for accuracy and completeness.
• Support orientation and ongoing training of care staff.
• Assist with scheduling in collaboration with leadership.
• Attend required in-services and continuing education.
• Maintain professional appearance and hygiene.
• Promote respectful and effective communication across teams and families.
• Perform other duties assigned to support resident care and operational excellence.
